Provides and documents in the patient’s electronic medical record a psycho-social assessment that utilizes critical thinking skills appropriate to multiple age groups, family dynamics, diverse populations, etc. Serves as a liaison between hospital team members, patients, families, community health, and social and governmental agencies. Provides on-site response within 60 minutes of being called for trauma/crisis support. Collaborates with interdisciplinary and community resources to meet the assessed needs. Performs psycho-social assessment and develops a support plan that includes sensitivity to the cultural and special needs of each situation.
